MNG is in the process of transitioning test ordering and sample routing to Labcorp channels. Over the next several weeks, Labcorp team members will work with you and your office to convert your services to Labcorp, with a transition date of June 13, 2025. 

MNG test request forms will no longer be available for download after June 13. Tests can be submitted using these forms until the transition date and will be subject to direct to MNG billing and reporting practices. After the transition date, Labcorp test request forms and test codes will be required to order testing performed at the MNG site. The test crosswalk is available upon request or through your Labcorp account representative.
